How to find EPO Provider 

Open Enrollment is the ONLY time you can enroll, add/remove dependents or make changes to your existing benefit plan choices without a qualifying life event (e.g. marriage, divorce, birth, adoption, etc).  *SO PLEASE DON'T DELAY!*
There are 3 ways to enroll:
  • NEW! Self - Enroll by logging on to the online enrollment platform from the comfort of your own home or workstation logging on to AFEnroll at  http://www.afenroll.com/enroll  BEGINNING ON NOVEMBER 1, 2016  
**NOTE: AFEnroll Online Site will not be available prior to 11/1/16*
  • Not comfortable enrolling yourself? Schedule a one-on-one appointment with a Representative from American Fidelity who will assist you with the online enrollment system.

There are no changes to the medical or dental plan coverage for 2017, however, monthly premium rates were increased on the Kaiser and PPO plans. Plan comparisons should be thoroughly reviewed to determine which plan best fits your personal situation, as changes cannot be made after open enrollment without a qualifying life event.    
Employee Costs for benefits can be reviewed based on the number of hours worked.  The rates are TENTHLY, and deducted from the September - June regular paychecks.   This amount is based on a composite rate, not tiered, meaning the rate covers the employee and all eligible dependents
If enrolling any dependents or spouse, please be sure to bring a copy of your marriage certificate to enroll a spouse, and birth certificate to enroll any children dependents.    All enrolled members MUST have social security numbers entered into system.   This is required by Medicare and by the IRS.   If you are terminating a spouse, you must have proof of dissolution of marriage.
